Coverage Territory: Focus on Hospital Admissions and building relationships with the hospitals in the area; also helping with Field Admissions and Visits as needed

\n \nAs a Transitions Care Hospice Nurse you are responsible for providing care to patients in need. The Nurse functions as a support team member to the IDG team by providing additional services when required. The Nurse will assess, address and report patient needs to the case management team all while ensuring that all patient needs are met in a timely manner. Local travel is required.
